Requesting a Death Certificate
Death Records are $22.00, which includes $13.50 in state fees. Certified copies are required for most official uses of vital records. They carry the seal of the registrar and are on special security paper.
Three (3) ways to request:
1. Order Death Certificates On Our Website

You can pay by credit/debit card by submitting an online request through our website HERE and accepts the following credit cards; Discover, Master Card or Visa. You do not have to have a Paypal account to process a payment.
2. Order By Mail
Send the form by U.S. mail: You can print out a request form Click here to download and print then mail it in with payment to ZMCHD, 205 N. 7th Street, Zanesville, OH 43701. The day we receive the form, it is processed and sent out in the same days mail. You will need to enclose a money order of $22.00 per certificate PLUS postage.

3. In Person At Our Office
Visit the ZMCHD office at 205 N. 7th Street, Zanesville OH 43701. Hours are Monday through Friday, 8am-4:00pm, but closed government holidays. This is the fastest way to obtain a birth or death certificate. Please Click here to print the instructions and application (complete two pages). You will fill out an application and pay the $22.00 required fee. Your request will be processed as you wait. Normal waiting time is 15 minutes or less. We accept cash, check, money orders (in U.S. funds), and credit/debit cards. No request will be filled if the application is not complete. You can also order a Birth Certificate by visiting any local health department in Ohio that issues birth certificates. Prices vary per local health department.

About Muskingum County Vital Records
Vital records include Birth Certificates, Death Certificates, Paternity Affidavits and Burial Permits. The Zanesville-Muskingum County Health Department (ZMCHD) maintains records of Births and Deaths occurring within the City of Zanesville and Muskingum County from December 20, 1908 to present.
Older records from 1867 to December 19, 1908 and Marriage Certificates, are provided by Muskingum County Probate Court, Courthouse, Zanesville, OH 43701. Call 740-455-7113 or e-mail probate@muskingumcounty.org. Prior to 1867, Ohio law did not require records to be kept.
Genealogy Research: We encourage use of the genealogy services at the Muskingum County Library System. We do accept requests through the mail and will return information requested as soon as time allows. No copies of documents are issued. If you require a legal document, a State certified certificate will be issued at a cost of $22.00. Please note that processing of current documents will take precedence over research of genealogy documents. Telephone inquiries are not accepted for genealogy research or verification.
