OHIZ Post Survey

The Zanesville-Muskingum County Health Department and partnering agencies continue to work within the greater Brighton, Eastside, Putnam and West Main areas on the Ohio Health Improvement Zone (OHIZ) Grant—a two-phase project that started in August 2022.

Your voice and feedback are important to the success of the project if you participated in one of the following activities throughout the course of the project:

  1. Completed the Community Assessment for Brighton, Eastside, Putnam and West Main,
  2. Participated in community discussions at Bethel Community Center, James Singer Center, Muskingum County EMA, or the Gant House and/or,
  3. Attended a monthly Ohio Health Improvement Zone Partner Meeting

The Zanesville-Muskingum County Health Department would appreciate your continued participation by completing a short, 15 question post-survey that aims to measure knowledge of and engagement with the Ohio Health Improvement Zone (OHIZ) project thus far.
